There are severals things to address when we want to change something (in general).
1. WHY: the first and fore most thing we have to address when we are considering change is the reason why we want that change. In my practice as a Plastic Surgeon, the Cosmetic or Aesthetic aspect of our services is considered "non-essential." Meaning, even if we don't provide the service requested, nothing bad will happen to the patient. Comparing it to other fields of medicine where if the disease of the patient is not treated may lead to deterioration (or at the extreme end of the spectrum, demise of the patient).
